 Is it best to have a watchtower at each entrance? or does one handle the whole city?

no You only get two per city so they will not show you all four entrances. you will need two of them and if you place them top center and bottom center it is possible to see all four entrances. if you pay 300 gold (150 each) to level them all the way up to level 5 you will create the "best" usage of them. refer to: http://1100ad.com/wiki/Gguide#Watchtower for the gold costs and images of each level.
